IMPORTANT INFORMATION - -PLEASE READ
New Patients - If you are interested in this free screening. - click on the left menu on FORMS and fill out the patient paperwork and then stop by our office at 124 N Ohio Ave. (across from the Post office). Mention the From Gray To Pink program and our friendly front office person will help you out and determine if you qualify.
Existing Patients - If you are already a patient of CCSC and are interested, please call ahead so we can fill out a referral form for you to take to your appointment.
If you are a current patient of Compassionate Care, stop by to pick up your referral letter for the 'From Gray To Pink' program to take to Wilson Health/Francis Women's Center. You must qualify as a patient of Compassionate Care to be referred to this program. Thank you.

It can be posted to your web sites, blogs, newsletters, etc., as a way to indicate that you are a valued member of our partner network. Please feel free to use the attached image to upload to your existing web site, using the instructions provided below.
Kroger has now made fundraising easier for Compassionate Care of Shelby County (CCSC) with the Kroger Community Rewards Program. Kroger no longer requires that you use a gift card to have your shopping support CCSC. All you do now is go online and register for ‘mykroger.com’ and then select Compassionate Care as the organization that you want to support. After you are registered it will be much easier to support us while you shop at Kroger. Below are the instructions. Our organization number is 81875. Please don’t hesitate to ask if you need help getting signed up, and feel free to share this with anyone who shops at Kroger.
Be sure to have your Kroger Plus card handy and register your card with Compassionate Care after you sign up.
If you do not yet have a Kroger Plus card, please pick one up at the customer service desk at any Kroger.
Click on Sign In/Register
Most participants are new online customers, so you must click on SIGN UP TODAY in the ‘New Customer?’ box.
Sign up for a Kroger Rewards Account by entering your zip code, clicking on your favorite store, entering your email address and creating a password, and agreeing to the terms and conditions
You will then get a message to check your email inbox and click on the link within the body of the email.
Click on My Account and use your email address and password to proceed to the next step.
Click on Edit Kroger Community Rewards information and input your Kroger Plus card number.
Update or confirm your information.
Enter 81875, select Compassionate Care from the list and click on confirm.
To verify you are enrolled correctly, you will see Compassionate Care on the right side of your information page.
REMEMBER, purchases will not count for Compassionate Care until after you register your card.
Do you use your phone number at the register? Call 800-576-4377, select option 4 to get your Kroger Plus card number.
Members must swipe their registered Kroger Plus Card or use the phone number that is related to their registered Kroger Plus Card when shopping for each purchase to count.
The gift card is accepted in all Kroger Affiliated Stores throughout the U.S. Kroger, Fred Meyer, Ralphs, Fry’s & Fry’s Marketplace, King Soopers, Dillons, Smith’s Food & Drug, City Market, QFC, Food 4 Less, Owen’s, FoodsCo., Hilander, Pay-Less, Cala-Bell, Baker’s, JayC, Fred Meyer Jewelers, Littman & Barclay Jewelers.
