How can you qualify to be a patient at Compassionate Care?
You must be a resident of Shelby County, Ohio.
You must have little or no health care insurance.
(We do not see people who qualify for Medicare, Medicaid, or who are eligible for Veteran’s Benefits.)
Your household must have an income at or below 250% of the Federal Poverty Level.
(For one person this would be an income of $29,175 or less a year. For more information you can visit

Q: What should I bring with me to my first visit?
A: New patients should bring
(Driver's License or State ID)
Proof of Residence
(utility bill, driver's license, piece of mail with your name and address)
Social Security Card
(1040 or 1040EZ)
